The REACH (Regenerating Environments and Communities’ Health) project has been funded by the Big Lottery Fund in the Southern Area for 5 years from 2010. 1. Aim and objectives REACH will empower and build the capacity of individuals, families and communities across the Southern Trust Area who are faced with inequalities in health through disadvantage, ethnicity or disability, to improve their health and wellbeing through interaction with, and improvement of their environment. SMART Outcome 1 By the end of year 5, 4625 volunteer days will have been committed in urban/rural areas to actively regenerate and improve the environment, with six community groups established and taking ownership of sustaining environmental improvement SMART Outcome 2 By the end of year 5, 2250 people will have participated in community lifestyle programmes and report increased health awareness, skills and improved health behaviour in support of positive health and wellbeing. SMART Outcome 3 By the end of year 5, 555 overweight/obese children and their parents will have participated in Family Weight Management Programmes with evidence of increased knowledge and skills to make improvements to family nutrition and activity levels. In addition, a further 500 people within families will have benefited from improved family nutrition. SMART Outcome 4 By the end of year 5, 672 individuals within the target groups have been supported to make lifestyle change through the support of 30 community health mentor volunteers. SMART Outcome 5 By the end of year 5, 1200 people in the target groups are regularly using the local environment to support their physical health and mental wellbeing. 2. Approaches In order to best meet the needs of target groups and the overarching aim and objectives of the project, REACH will incorporate the delivery of the following activities and services: Green Gym – this distinct element of REACH will offer individuals and groups through practical conservation work, the opportunity to improve and enhance health and wellbeing and improve ownership and involvement with the living environment Family Weight Management Programmes - Holistic family orientated programmes will be offered in community settings to families with overweight and obese children. Community Volunteers will be trained as Community Health Mentors - Community Health Mentors will provide information and skills within the communities to offer peer support and help people overcome barriers which hinder a healthy lifestyle. Community Lifestyle Co-ordinators and Support Workers – These posts will facilitate healthy lifestyle assessment, and education programmes and activities tailored to local community need.
