Directorate Background

The Directorate of Older People and Primary Care Services are managed by Dr. Rankin and delivers services through four Divisions:

·          Older People,

·          Primary Care,

·          Enhanced Services,

·          Promoting Well Being.

Older People

This Division oversees the provision of a domiciliary care services aimed at supporting older people to remain at home even with complex care needs. Carer support and respite care options are in integral part of the service.

Primary Care

Primary Care provides a range of health and social care services aimed at helping people to remain within their own homes. Provided in or close to a person's own home, Primary Care services offer a broad range of nursing care including district Nursing, diabetes services and continence services. In addition, Allied Health Professions (AHP’s) provide clinic-based therapy, assessment and treatment in domiciliary and residential care settings as well as equipment needed to help people remain as independent as possible within their own environment. These services work closely with General Practitioners and with a range of local voluntary and community organisations in order to meet individual needs.

Enhanced Services

Enhanced Services provide a wide range of services including Specialist Nursing and Chronic Disease Management, GP “Out of Hours,” Intermediate Care, ICATS and Non-Acute Hospitals provision in South Tyrone, Lurgan and Mullinure Hospitals.

Promoting Wellbeing

The Promoting Wellbeing Department is responsible for the development and implementation of action to improve health and wellbeing and reduce health inequality. In addition, the Department has the lead role within the Trust for PPI agenda. The Promoting Wellbeing staff work in partnership with Directorate teams, communities and partner organisations to identify needs and priorities, develop plans, programmes and actions to address these, co-ordinate action to address area and regional priorities in a local sensitive manner and ensure effective evaluation, monitoring and reporting of this work across the Trust.
